DLOOKUP Same Field

I know my problem is probably simple, but I tried that and it doesn't work.  So I went through everything and wrote it down exactly as it is in my database.  This is what I have!

Why do you need it in two places?
I still think you should have it in 1 place and then link the Request to the Employee with an EmployeeID in the RequestF and set that to have a Default Value of [Forms]![EmployeeF]![EmployeeID] so when you open the Request Form from the EmployeeF it sets the value to that Employee, then you can fill in the extra information you need.
