Medical Database

Hello.
I want to build a ficticious Medical Database for the sake of further expanding knowing how to build databases.
I curently have a table for Doctor Information and one for Patient Information. In the Patient table, I am unclear on the data that sould be in that table. Do I make a different table for medical history and so on. In my mind, I keep thinking all the data for 1 record should be viewed in one form, or am I wrong? See all at once?

I would have a separate table for the patient's medical history. You could either open a form via a Command Button on the Patient form or display the data on the same Form with a Subform. Either way of displaying the medical history is completely valid. It all depends on how you want your database to work, and the look and feel you are going for.

I think this is a perfect scenario to use a many to many relationship.
As a patient I might have many Dr.s but the history is all mine.  The Dr.s treat many Patients.

Double click on an ID to bring up a VisitDetailF form containing extended details on the particular visit.

I have been working on such a database for years.  I have tables for doctors, patients, prescriptions, history, vaccinations, dental visits, etc.  I have changed it numerous times over the years and requirements changed.  To answer your question: Make separate tables for each item you want to track.  Gives you more flexibility in the future.

Get your index cards out and write down all the information you think you need.
Then work out what can live where and try it out

My Dad and Mother are both Doctors and I've worked in their offices growing up as a teenager.  I've seen so many types of software go through their offices until they finally found the right one to keep track of 50+ years of combined services between the both of them.  These are just a few of the Records that their patient files kept:

- Personal Information
- Visits
- Prescriptions (Medication)
- Prescriptions (Medical-Equipment)
- Symptoms
- Other Doctor's
- Nurses Seen (Day Nurses, Travel Nurses...)
- History of any health emergencies (i.e. broken bones).
- Childbirth history.
- Vaccination history.
- Health screenings (i.e. blood pressure, cholesterol and blood sugar readings).
- Cancer screenings (i.e. pap smears, mammograms, colonoscopies).
- Blood and urine test results.
- Results from MRIs or X-rays.
- Surgery history.
- Hearing and vision test results.
-Info on any current/former medications or supplements.
-Medical bills and insurance information.

Again, these are just a few out of the hundreds of other things they could check for.  One thing you could do is ask your own physician on what they keep track of, I bet you that would be one sure way to get all the information you are looking for.
