I have tried to compact and repair, and shut down Access and restart, I've moved it to different folders.  I'm stumped, this is the first time ive encountered this.

From ChatGPT:

This means Access tried to use an external program (Excel, Word, PDF viewer, etc.) or an ActiveX control that didn't respond correctly.

Common fixes:

Restart the OLE server
Close Access and any related programs such as Excel, Word, or Acrobat.
End any lingering processes (like EXCEL.EXE) in Task Manager, then reopen Access.

Check ActiveX controls
Open the form in Design View.
Re-add or replace any ActiveX controls that might be corrupted, or re-register their .ocx files using the Windows command regsvr32 followed by the file path.

Recreate corrupted forms
Copy the controls to a new blank form and delete the old one if the problem goes away.

Clear temporary files
Close Access and delete all files in your %temp% folder.

Check VBA references
In the VBA editor, go to Tools  References and remove any that show as MISSING.

Compact and repair / Office repair
Run "Compact and Repair Database" in Access, and if needed, perform a Microsoft Office Quick Repair.

Release OLE objects in VBA
If your code automates Excel, Word, or other apps, make sure you properly quit and set those objects to Nothing when done.

Tip: The error is often temporary - restarting the OLE server or Access usually clears it up quickly.

You most likely have a bad or missing reference as this was noted above...
