Form SubForm based on Multiple Criteria

Richard,
I don't know how to describe this, but here goes.  It's probably very simple but I just can't find it.
The Speech Dept treats very young children, sent to them by the county.  The county wants us to use a form, and it cannot be modified in appearance.  The form is filled out for each month's activities.
The form has a large header, lots of fields particular to each child.  The field info does not change from month to month.
Below this header info are the notes recorded by the therapist:
M/D/Yr    TimeFrom  TimeTo  Notes
Every call made on the child's behalf is recorded, with the amount of time spent each day subtotaled.
I put this in Excel because they LOVE the subtotal function. But!  Excel has a 255 character per cell limit.  There notes get rather wordy.  This is why I thought Access would work great.

Here is the problem:  in the header is a field called "Month Notes Covered By:".  They would write "February 2012" here, and enter each date below.

I have a ChildT and a MonthlyNotesT.  With a ChildF and a MonthlyNotesSubForm.  The problem is the Month Notes Covered by field.  It doesn't belong with the ChildT info. Including it in the MonthlyNotes info seems redundant.
And, I need to be able to call the notes for the child in question AND for the month in question.  And, have the Month Notes Covered by field in the header.

I'm really stuck on calling info based on 2 fields, and getting it in the header.
I apologize for the length.  I just wanted to be as clear as possible.
You help would be appreciated.

It sounds to me like you just need to include a date field in your MonthlyNotesT. Correct me if I'm wrong, but your data looks like this:

ChildT
ChildID, FirstName, LastName, etc.

MonthlyNotesT
MonthlyNotesID, ChildID (Foreign Key), DateTimeOfVisit (Date), Notes (Memo), etc.

That should do it... or am I missing something?
