I'm using Richards track changes code to make a log table.  It works but the log table will save the first entry, then when it saves the second entry it will repeat the first and second.  Then the thrid entry will copy all three again.  It gets nuts.  Attached are the screen captures.  What am I doing wrong?  Thanks in advance.
