What information will be on the form?  If it's personal identification information I would not give it to a business whose model is selling peoples information.  I treat google as evil.  I dont trust them as far as I can throw them with information like this.

i was hoping to send each client a personal form based upon their business activity statmets, or tax returns, etc, with their particular bank, credit card, & loan stateents, with question boxes,  and file upload requests.
After reading your post i realise i could accomplish this with adobe pro. Unless you have a better method
