Say I have frmMain, frmA, frmB, frmC, and frmD. As I enter info on frmMain, I have a drop-down menu with four choices (A, B, C, and D) - once the info is entered and a choice made, I want click on a single button that will take the info from frmMain and enter it onto a new record of the appropriate form.

frmMain contains basic info (i.e. Name, SSN, DOB, etc.) of a patient, and the combo box lists the four possible procedures to be done.

frmA is Procedure 1, frmB is Procedure 2, frmC is Procedure 3, and frmD is Procedure 4 - in tblProcedure, I have the ProcedureID with the corresponding ProcedureName and ProcedureForm.

How do I make this happen? Or is there a better way to complete this task?

I would use VBA code to open the form and then set the values.

Docmd.Openform "FormName"
Forms!FormName!FirstName = Forms!Main!FirstName
etc.

However, I have to ask WHY you want to put duplicate information into four different forms/tables?
