Cascading Combo Box and Export

I am creating a database to track some cellular testing that my team has been doing.  I have tables for the test event, test location, and one for the actual test data.  I was recently given a the task of exporting the data for the previous month  into excel
so that it can be uploaded to someone elses database.  I created a query to pull all the data together and it pulls its
event ID from an unbound form. This form contains just four fields (event ID, event city, event start date, and event end date).  The City and
start date are combo boxes.  I can actually get it the query and the export feature to work (sort of) by selecting the city and start date.

The problem with the way it is working right now, its only pulling test data from the date in the start date combo box.  It does properly filter the
city.  It also does not populate the event ID field.  What I need is a way to have the form feed the query what it needs to provide all the test datafor
said city, for the full duration of the event.

The second issue that I have noticed is that the resulting excel file is not formmated completely correct.  Even though I put the
fields into the query exactly as they need to be in the excel sheet.  It takes a couple of the fields and moves them to the end.

My goal is to have a cascading combo box for the city and start date and some sort of lookup to populate the ID and end date. then when clicking on export, I get a properly
formated excel file.  Can this be done?
