I've seen this kind of behavior myself recently too. There's also been some discussion among the Access MVPs about continuous form display/rendering issues, so you're definitely not the only one seeing weird behavior like this. Microsoft is aware that there have been some issues in this area. I wouldn't spend too much time tearing your database apart trying to find something you did wrong. Hopefully we'll see it cleaned up in an update soon. I'll post more about it as soon as I hear more information, but you're not alone. I'm seeing this in my databases too.

Yeah, me too. I've got one task management database that I use every day, and now whenever I add a new task and requery the list, I see the task twice. One of them is a little off-center, and it's like, "What the hell is happening?"

I went through all the normal troubleshooting steps and eventually searched some of the Access MVP email groups. It's been reported before, so Microsoft is aware of it. Like any developer, whenever I see something weird after changing something, my first thought is always, "Okay, what did I break now?"
