I'm using Access 2013 and I've created Db for my Hypnotherapy business to keep track of my clients visit. I've got table named(amount others) ClientsVisits with the fields: Wellbeen and Progress, in which will clients rank those attributes on  a scale 1-10 in every session.The idea behind this is to collect the data of client't progress and display them as a subform in Client's visit form as a simple graph. This way I would get at one glance, overall picture about the progress of every client. Any idea how to go about this? The way I was thinking about this was, that I need to somehow generate  client's visit No. This would be on the bottom axes of the graph with above mentioned attributes to be displayed on left axes. But I don't know how to generate visit number, which would be different for each client, but still be in incremental order.   Any help would be greatly appreciated.

You have a PatientT table for your parent form, then a VisitT table for your child form (subform). It's a basic one-to-many relationship, covered in Access Expert 1. I'd use a combo box for each of your fields that your patient can select a value from. Creating an actual CHART is just a matter of plotting that data in a form or report chart.
