Using MS Office 2003: Expertise-beginner by my standards but learning quickly.

I have a DB that tracks student behavior infractions with the back end installed on school distrcit server, front end installed at each individual school.  All tables are in back end DB except the student table which has fields for student name, race, gender, and grade, all field specific to the school location.  This table is one of many Lookup Tables for data input into the main Referral table through a form.  I'm having trouble setting this up so that the teacher can enter the student info once into the student LUT, select from the list of students in the Referrals form and have the name, race, gender and grade automatically entered into the referrals table from that selection.  What am I doing wrong?  Does that have to be done in a module?  Or can it be done through a macro?  Your help would be greatly appreciated.
