From the rental video I can not seem to get this to work.  Been looking at it for 3 days and trying different varations but seems as soon as I put the 4th select item I get the runtime 3075 error.  Any assistance please and thank you

What about

CurrentDB.Execute "INSERT INTO IPHistory_T (IPAddressID, DeviceID, EnvironmentID, IPNotes) VALUES ('" & IPAddressID & "','" & CurrentDeviceCombo & "','" & EnvironmentCombo & "','" & IPNotes & "')"

Before DoCmd.RunSQL S add a Debug.Print S, open the Immediate Window and check the output, then share here. (Removing anything sensitive)

You're sending the VALUE of field names, not the field names themselves, to your SQL

Adam and Kevin's worked.  Although a little confused on Adams, as it appears you are using "Double Quotes" for all fields, with the single quote?

Kevin I used the below quotes around the string field IPNotes:
    S = "INSERT INTO IPHistory_T (IPAddressID, DeviceID, EnvironmentID, IPNotes ) " & _
    "SELECT " & IPAddressID & ", " & CurrentDeviceCombo & ", " & EnvironmentCombo & ", " & "IPNotes" & ""

Yes I was, I didn't know if you were storing a string value or a number so I put them on all of them, you'd just need for the string fields.
