I'm guessing that you set the form as a background.  
Click on the form picture and then right click > Position > Send to back.

If that isn't it I don't know what you did.  Not enough information here to help further.

Do you think this might be related... I noticed in the second image that the page isn't just BLANK. It says "_of_" records in the navigation bar.

My  form image was in the back and I redid it to confirm. I do have records to display it. I know i will figure it out eventually . I have to as this is imperative for our billing. thanks,

I see in the first property sheet image that your record source is CMSINFOENTRY
1. is the spelling correct?
2. are there records there?
3. is there a record id 2 in there?

The reason I ask #3 is there is a filter in the property sheet.  Filter     [ID] = 2
That could possibly be your problem.  Try removing that.

Yeah it is usually those small details that get ya!
The very first thing I do when something doesn't work that I expect is check for #1 and #2 above.
