Technical Help: I've been trying to understand this for a week, if someone would be able to shed light on what I'm doing wrong or how this should be structured.

Desired Outcome: Run a Report for a Case File. Listing multiple Auto Insurance Carriers and their coverage for a Plaintiff and a Defendant, and multiple Medical Providers (Reference Form Picture), all of which are related to one case file report.

I've created Junction tables to capture the values for the Auto Carrier, Type of Insurance, and Coverage Amounts, Medical Providers etc.

Issue: When I run the report I am displayed with duplicative records.

I'm not sure how to get the details on the Report to separate as well, so that I have a list for each category (Defendant, Plaintiff, Medical Providers etc.)
