Tabe oops I didn t see coming

So I'm working on a database for our speech department.  They treat alot of children sent to them from the county.  They county assigns each child a unique ID number.  All communication from the county only contains this ChildID, so as to preserve their privacy.
So I set up the PatientT with PatientID ChildID LastName, FirstName, Address, blah, blah, blah.
Then I made a Parent/GuardianT, thinking each child can have 2 parents,and if social services is involved there could be an additional guardian assigned by the courts.  I also made a RelationshipT to identify how each adult is related to the child: mom, dad, stepmon, mother's partner, etc.  The fields look like this:
ParentID, PatientID, RelationshipID, LastName, FirstName.  The link was PatientID to PatientID because I was always thinking "one child with multiple parents".
Here is my quandry:  I now have 2 to 3 children from the same family receiving services!!!  So now I face entering the parent info multiple times in the same table so I can assigned the multiple children to them.
Is there a way to fix this (easily)?
I was thinking I could go back in the PatientT and enter the ParentID field as a foreign key.  Then change the link to ParentID (I have, in a form, the ParentF as a subform in the PatientF).

hope I have explained this clearly.  Please let me know if I can clarify things better.  And thanks for your help.
