Alex, Thank you for your quick reply.  I don't quite get your answer.  I re-read my post and saw I mis-lead you.  My fault.

After looking this over I think I have a many-to-many relationship thing going on and may need a juction table (and I have no clue on doing one).  Each child can have "many" parents, while each parent can have many children.  What do you think?

To clear up the layout of my tables:
PatientT has;PatientID, Lastname, Firstname, other stuff
ParentT has;ParentID,PatientID,RelationshipID,Lastname,Firstname
RelationshipT has only RelationshipID, Relationship.
When there was only one child in the family receiving services, I linked PatientT.PatientID(PK) with ParentT.PatientID(FK).  All was well.  Each child would show one or more parents.  Now, not only does child Bob Smith have parents Jim & Sally, but so does his brother Billy!
I'm very confused at this point.  I'm considering the career in knitting again :)
