2 listbox controling one form

I have 2 listboxes, one with Anniversary dates the other with DOB. The listboxes are configured to display the address of the contact selected in a form for each list when selected. The list boxes are using the same query to display the address. I am using an OR statement in the NameID criteria for list. My problem is it works intermittently. The first click displays the address perfectly from either list however after that, it does not. i did not want to have 2 address forms for each list but i cannot get it to work properly, no errors are displayed.

I'm not sure I understand the use case. Why would picking a date choose an address? Can you share an anonymised screenshot of your form layout?

Sorry i didn't explain the issue very well. There are 2 list boxes, the user has asked if the anniversary list as well as the Birthday list can output the address of the contact selected to the address form on the right. It works well from one list to the address form however when i add the second list to the criteria of the query it works once. This is what i expected it to as i am not sure it is possible for 2 different list boxes to output to the same address form. I used an OR statement between the 2 criteria's. See images
