Hi Richard. The form, MOP, will need to be able to be printed and emailed. What the form will look like is Site Name, Address, City, State, Zip, Site ID, Site Type, Contact Personnel, Site requirements for notification. Each site has specific things that need to be the same everytime this form is made for a new task at that site. What is currently happening is people are entering everything by hand in a Word document but they are either leaving old data from a copied MOP or leaving data out or putting the wrong names and numbers in causing a lot of rework and professional embarrassment. I voulunteered to learn Access and create this form and I am sure more automated forms down the road. Any help you could provide is very welcome!

Yes, my courses can teach you what you want to know, but picking a value from a list (your Sites) and having that information automatically fill into another form requires a tad bit of programming. I do cover this in my classes, but not until my Access 302 class - my advanced series. I show you how to select a product from a list and add it to an invoice - same basic concept... you're copying data from one table to another one using an AfterUpdate event.
