Access cannot detect you making changes in design view.  There are no events associated with that.  Access can only detect you entering records on a form, clicking buttons, that sort of thing, because Access has events for those.  But there are no events for you making design changes -- adding controls to a form, resizing a textbox, etc.  Either Microsoft has to add this as a built-in feature (slim chance of that happening), or you need to take notes yourself, using a note-taking app like OneNotes.
