Pull data into form from one table save it to ano

Hello,
I know the subject sounds kooky but here I go.
The situation is we have cardiac patients coming in  for rehab.  Classes are held hourly. There are a set number of slots, and patients rarely ever change their time slot.
So I was thinking of having a table of patients with:ClassSlot, ClassTime, PatientName, MedicalNote. This info wouldn't change until their discharge.
My problem is this: to minimize data entry, I wanted to be able to open a form and show all the above info, but also have a few unbound textboxes so I could add a date, and a No Show/Cancellation field.  Then save ALL this info to an "attendence" table (so to speak) so I can keep a record of everyone coming to class or not.
Any ideas on how to go about this?  Your help would be most appreciated.

Ideally, you would have a Patient table with just the patient info. Then you would have a Class table with the details for each class instance (slot, time, etc.) and a PatientID to know who's in each slot.

Now, as part of your Class record, you could have an attendance field which is just another combo box to store that result too. Two tables should do it.
