Do you mean "automatically" in that the moment they walk through the door a facial recognition system identifies the client and docks them one visit? I'm joking.

I assume you have a Customer table with basic customer information and assume you need a second table to keep track of appointments that might have scheduledtime of appointment etc. I would create a third table that linked to appointments that held information on charges, payments, dateOfPayments, TypeOfPayment etc. You could handle the prepayments a couple of different ways.
