Richard, I followed your checklist video to create an employee performance review checklist, the only thing I am having an issue with is trying to add the EmployeeID to the form. I created an updated query but the only way I know how to update it is placing a [Enter EmployeeID] in the update To area in the query. Is there anyway I can have access enter this automatically?

Are you trying to add an employee ID to be chosen for every item on the checklist? If that is what you're trying to do, all you have to do is insert a combo box that selects the employee from the employee table. See this Combo Box and Relational Combo

Thank you for responding, yes I’m trying to add the employeeID to every question, but I’m opening it up the checklist under an employee form . The query I made does work, but I still have to manually add the employee ID

You do not need a query for that. You just need a combo box that is bound to the employee ID.
