Albeit much less likely, I worked on a database for someone about a year ago. The person who had designed it had left the company. The manager knew how to do "basic" changes in the developer's version and push updates to the front ends throughout the facility.

One design "feature" in the database had been that everyone looked at basically the same thing. If you could see and open most forms, you would see the same things as other people, but you could only edit what your job allowed. Sound simple enough - just like described in this video...

However, instead of just making forms read-only, the developer had made copies of the forms as reports that would open when the button or link was clicked in place of the form so there was no way they could be edited.

Extremely rare but, if someone who only does database consulting occasionally, like me, can run into it, others may too.
