If i want to make one table as master table and i want many sub table on it

what kind of relationship i can use?

For example, the main table will be (Patient information)
The first sub-table will record (Track visit Patient date)
The second sub-Table will record (Type of service has done for patient and linked with (date of visit in the first sub table )
The third Table will be linked also and that will give next appointment to the patient

The main goal for me is to link many sub-table under primary table
