What version of Windows are you using Rodney?
Richard is on Win 11 and they have made some changes in Windows on how this appears.

I am on Win10 22H2 with all the current updates as of today
I have Office Professional 2019 32bit because a 64 bit access Db gives error cannot open.

I'm also on Win 10 still using a Office 365 subscription.
The check boxes are purely cosmetic.  Screen shot below.

As far as the sample db goes I'm not having any issues opening it to date.
There isn't anything in this db, as of right now, that I have found that won't work in 32-bit.  YMMV.  I know that Richard I know has switched over to 64-bit but I'm not having any issues with my 32-bit install on this particular sample db.

In your screeen shot it is when family is null and double click that it get the error message
Otherwise everything works for me

Before the line to open the form add this line:
If IsNull(FamilyCombo) Then Exit Sub
