No. Forms are for data entry. Reports are for printing that data.

Are you asking if you can send a report, as a PDF, to somebody else and have some areas that can be typed in?

Hi John
I remember that Richard once created text boxes that overlaid a Form and used that like a fillable form. It was very cleverly done and I will see if I can find it.

Yes, John. I have done something similar by taking a screen shot of a report with empty textBoxes and using this graphic as a background for a form, placing properly sized, editable textBoxes on the Form in strategic places making them appear to "float" over the back PNG.

One possibility is to create a Survey in Google to gather your data.  Surveys

If you are open to a third party software I've used Power PDF - now owned by Kofax for years.  Basically create a form in MS Word. Add in your Fields then convert to PDF. (not quite that simple but close)
You can then have people fill out the data and send it back.  Once you get it, you can export the data and import to your db.
More involved but you can layout and make a good looking doc that gathers the data to import.

Richard's done a million ways to do different things. (OK, maybe not a million but a lot).
Take a look at the Data Collection video also.
