Did you add the Input Mask after you created the form? If yes, you would have needed to have clicked on the Property Update button to update the field on the form. There is also an Input Mask property for the form field (on the Data tab).

Thanks guys, but I did figure it out by deleting the field (phone number) I copy another table and did it directly from the design view and it works.
