My apologies, the video contained mutliple-field search the text fields for FirstName, LastName, State and the rest of the fields worked ok with the search form, but the checkbox for the "IsActive" field would only show the "Active" records when the IsActiveSearch box was checked or Null, and no records for the records that where unchecked or not active. I do have records with inactive records.

Ravi the question is better but there still isn't enough information to help you out.  Since you posted this under the "General" Developer Forum we're not sure which video you are referring to.

In addition, we will need you to show us what you have so that we can guide you how to fix it.  See Image Uploads. If you are unsure of how to create a screen shot there is a link in that article for help there.  

Are you getting any error messages?

Hi Scott, thanks for replying. it was the "Search 2.0" Video. I do not get any error messages. I will Upload the form, the moment i get a chance to.
